Why it exists

Most product processes fail the same way.

Not because teams lack talent or effort - because the knowledge is scattered. Validated insights that never made it into the spec. Business rules buried in Slack threads. A PRD nobody reads after week two. A new developer who spends three days asking basic questions about how the system works.
